The Calvert Hospice Clinical Team

No one person can provide for all the needs of hospice patients and their families. Hospice care at Calvert Hospice is provided by a team of people with different specialties who work together to provide comprehensive and responsive care that benefits both patients and families. The team consists of the following people:

Nurse

Registered Nurses provide care to patients in their homes. Nurses provide physical care and keep the doctor informed and updating regarding patients’ conditions. They also teach families how to care for patients. The frequency of nursing visits is based on the needs of the patient and family.

Social Worker

The Social Worker provides counseling and support to patients, families, and/or survivors, assisting them with questions and concerns related to the death and dying process. The Social Worker is also the primary contact for community resources and financial benefits (Social Security, Medicaid, etc.).

Hospice Aide

Hospice Aides are Certified Nursing Assistants who are available to provide patients help with bathing, dressing, and other types of personal care. They may also prepare small meals or provide basic tidying services to benefit patients and caregivers. The frequency of Hospice Aide visits is based on the needs of the patient and family.

Volunteer

Family Service Volunteers are trained to provide support and companionship to patients and their families. Volunteers can provide assistance in many ways, including staying with patients for short periods to allow family members to do other things, running errands, or providing transportation. Volunteers also assist in the hospice office, work with bereavement programs, provide pet therapy, and staff the Shoppe for Hospice.

Attending Physician

The patient’s regular physician, also known as the attending physician, is a key member of the Calvert Hospice team. The attending physician is responsible for managing the patient’s treatment while the patient is in the Calvert Hospice program.

Medical Director

The Medical Director is a physician who works with the nurses, other team members, and with the patient’s attending physician to provide added expertise for the patient’s care. The Medical Director may also manage the patient’s care in the absence of an attending physician.

Bereavement Coordinator

The Bereavement Coordinator provides grief support and counseling to family survivors to assist them through the grief process. We offer many programs to support both children and adults. The Bereavement Coordinator is also available to help family members not living in the area find resources where they live.
